Where each total sits

These are the instrument's own cut-offs, not this site's opinion of them.

PHQ-9 totalBandWhat it usually means next
0–4MinimalBelow the screening threshold. A total here does not point to depression. If you feel unwell anyway, the questionnaire is not the last word — say so to a clinician.
5–9MildSome symptoms most days. Usually watched rather than treated straight away: take it again in two weeks and see whether the number moves.
10–14ModerateTen is where most clinics start a conversation about treatment. Worth taking to a doctor rather than waiting.
15–19Moderately severeActive treatment is normally discussed at this level — talking therapy, medication, or both.
20–27SevereThe top band. Do not wait for a routine appointment. If item 9 was anything other than "not at all", treat it as urgent — 1122 in Pakistan.

04What it cannot do

The limits of a number

  • It does not diagnose. A total sorts people into bands of likelihood. Diagnosis is a clinical judgement made by a person, with history and context.
  • It cannot tell depression from its look-alikes. An underactive thyroid, anaemia, untreated pain, bereavement and burnout all raise the same items.
  • It measures two weeks. A bad fortnight and a five-year illness can produce the same number.
  • It does not know who is answering. Somebody filling it in with a relative reading over their shoulder answers differently, and the total is quietly wrong.
  • One item is not a screening question at all. Item 9 asks about thoughts of being better off dead. Anything above zero there needs a person, not a page.
